ESSENTIAL FUNCTIONS AND PERFORMANCE REQUIREMENTS: Reports problems and issues related to care, well-being, and health management of campus residents to the Director of Care Management and At-Home Program, the Vice President of Health Services and the Campus Continuing Care committee and recommends solutions and methods for improvement. Works collaboratively with team to develop departmental goals that promote La Posada’s mission, ends, and strategic goals, focusing on service excellence and exemplifying the values established by La Posada. Collaborates with hospital staff, community physicians, other healthcare providers involved in resident care, and works in tandem with the La Posada Medical Director to insure best care and services of residents. Represents La Posada to the community in a professional and positive manner. Conducts pre-move in health and cognitive assessments, screens residents as needed for medical necessity, evaluates appropriateness of the care setting, and recommends the appropriate level of care for each resident. Communicates appropriately to the resident, their family members, and to administration. Develops, plans, and implements resident, employees, and community wellness and education programs. Monitors and facilitates the utilization of wellness programs and maintains data. Anticipates and identifies variances in the care process related to the individual resident’s needs and facilitates pre and post-hospitalization follow-up, contacts with physicians, office staff, referral agencies, and family members. Conducts follow-up hospitalization visits of campus residents. Coordinates plan of care for residents requiring temporary homecare support in independent living. Oversees homecare caregivers. Supports and works in tandem with La Posada Homecare department and staff. Coordinates outside homecare for those residents qualifying for Medicare homecare. Maintains compliance with all regulatory and licensure requirements for homecare and maintains a good working relationship with inspecting and regulating agencies. Monitors resident satisfaction on a continual basis. Monitors care and service for At-Home Program as well as satisfaction of care and service from residents discharged from the At-Home Program. Investigates and recommends cutting edge developments on care related to serving the elderly. Assists with developing, monitoring, and maintaining integrity of department budget and participates in the development of and review of new and existing case management tools. Assists in oversight of caregiver staff in At-Home program. Evaluates performance and competency of care provided to residents by caregiver staff as needed and reports concerns or deficiencies observed to At-Home Program Coordinator and Director of Care Management and Homecare. Works independently and displays a kind, courteous attitude and works in a team-oriented setting with residents, relatives, staff and others. Develops and maintains a good working rapport with inter-departmental personnel and other departments throughout the campus. WORKING CONDITIONS: Able to work in resident apartments and homes as required. Works in a comfortable air-conditioned work area. Possible exposure to bruises, cuts, minor injury. Job duties may involve occasional exposure to blood, body fluids or tissue during the normal work routine. Training and compliance with dealing with blood borne pathogens is a requirement of the job. MINIMUM QUALIFICATIONS: - Registered Nurse.
- Minimum of five years of clinical experience.
- Current RN license with the State of Arizona or Compact Licensure.
- Demonstrated ability to communicate and coordinate a multi-disciplinary team effectively with patients/residents, family members, other healthcare providers, and visitors.

- Basic computer literacy.
- Demonstrated ability to anticipate and adapt to change.
