Essential Functions Statement(s)

  1. Treat mental and emotional disorders through assessment, diagnosis and psychotherapy with individuals, couples, families and groups.
  2. Assists individuals, families, groups and communities to enhance or restore the ability to function physically, socially, emotionally, mentally and economically.
  3. Provide community members services in their home or in office. Provide assessments, information, and referrals.
  4. Counsels residents/clients and work with their family members on concerns such as relationships, financial issues, medical conditions, loss, depression, and grief.
  5. Develops support groups as needed to address varied emotional and psychosocial issues throughout the continuum.
  6. Develops care plans for residents/clients being counseled and maintains files that include activities, progress notes, evaluations, recommendations, medications, and referrals. Provides one to one counseling as appropriate.
  7. Refers residents/clients when necessary to the appropriate individuals for treatment and/or medication, to clergy, or to gain help with legal, financial, or other personal issues.
  8. Contacts doctors, other social workers, counselors, clergy, law enforcement personnel, family members, or powers of attorney when appropriate or required.
  9. Works collaboratively with other organizations and service providers in delivering mental health services to clients, accepting referrals from such providers as appropriate.
  10. When required, remains available to accept phone calls. Will briefly assess the caller’s presenting issue(s) and make appropriate referral(s).
  11. Participates in community outreach activities designed to promote the services and resources available through Casa Community Services.
  12. Within the community, conducts educational presentations regarding mental and behavioral health.
  13. Is aware of and adheres to all HIPAA and privacy laws and is respectful of client/resident wishes unless the safety and/or well-being of the client/resident or others are at risk.
  14. Follows up on the results of counseling programs and client/resident input to determine the effectiveness of programs.
  15. Other duties as assigned.

 

The duties listed below would apply specifically to work performed for Case Management and Home Care:

  1. Works collaboratively with the case management team to coordinate the care of residents throughout the continuum on the La Posada campus. Develops relationships with residents in order to be able to proactively identify issues.
  2. Meets with new residents within thirty days of move in to establish a relationship and document emotional status and psycho-social needs, if applicable. Assesses residents as needed during the year.
  3. Advocates and educates residents and respective parties on the continuum philosophy of multiple service locations. Assists in the movement of residents through the continuum, helping residents and families to make emotional and mental preparations for change.  Provides follow up visits and support after relocation.

 

MINIMUM QUALIFICATIONS: 

  • Licensed as a Clinical Social Worker (LCSW) required.
  • Master’s degree in social work with fundamental knowledge and skills in counseling, social work assessment and intervention.
  • Minimum of three years of related experience in social work or counseling of adults. Has experience in working with older adults.

  • Demonstrates consideration and sensitivity to the feelings of others.
  • People-oriented individual, skilled at problem solving, conflict resolution, and consensus building. High level of communication, both written and verbal, and excellent interpersonal skills, including a good sense of humor.
  • Ability to make critical decisions while following company policies and procedures. Has the ability to formulate a sound decision using the available information.
  • Accepts responsibility and is accountable for his/her actions.
  • Conducts self professionally and conforms to La Posada’s set of values and accepted standards.
  • Is organized, proactive, and follows a systematic method of performing daily tasks so that other team members can assist with or follow the process used.
  • Computer skills – able to enter assessment data and chart notes as well as type follow-up letters as needed.
