We are excited to announce an incredible opportunity for a full-time Director of Case Management (RN) to be part of this exciting new chapter, working alongside an experienced and passionate team.

Position Overview:

Reporting directly to the Executive Vice President, you will lead a dynamic team of RN Navigators, Caregivers, and Social Service staff, all focused on enhancing the well-being and health management of our residents. In this key leadership role, you will collaborate on assessments, planning, facilitation, and advocacy to meet each resident’s unique health needs. Your expertise will guide the development of innovative programs, drive performance improvement, and coordinate the progress of residents, all while aligning with La Posada’s mission and strategic goals. You will also provide direct services and conduct off-campus assessments as needed, ensuring we offer the highest level of personalized care.

 This person must have the following skills: 

  • Communicates with the VP Health Services and the Campus Care Coordination team (CCC) and the Executive VP or CEO, regarding problems and issues related to care, well-being and health management of campus residents, and makes recommendations for improvement.
  • Promotes the La Posada mission; commitment to service excellence, values and ends; assists in developing department goals that support these and La Posada priorities.
  • Facilitates pre- and post-hospitalization follow-up contact, which may include the resident, family, physician, office staff and referral agencies.
  • Evaluates the appropriateness of the care setting, and screens for medical necessity and continued stay at level of care.
  • Recommends new programs to enhance resident care and well-being.
  • Monitors and facilitates the utilization of wellness data and resulting programs.
  • Recommends and is a team leader for campus resident wellness initiative, and assists in development and implementation of resident/community wellness and education program.
  • Campus liaison with contracted health plans as advocate for campus residents and to ensure appropriate and best use of resources.
  • Assists residents in the safe transitioning to the next most appropriate level of care.
  • Supports the VP Health Services and Executive VP in maintaining specific, measurable, attainable "big picture" goals. Reviews at least annually during planning and budgeting process, assisting with and creating, monitoring, and maintaining integrity of department budgets.
  • Responsible to assist in recruitment, training, supervision, developing, managing, and evaluating RN Navigators, care and support staff.
  • Monitors resident satisfaction on a continual basis. Works with team to address specific dissatisfaction promptly.
  • Investigates and recommends new developments in caring for the elderly.
  • With respect for the dignity of each resident, coordinates the appropriate placement in the continuum, recognizing that sometimes safety considerations must take precedence.
  • Participates in and /or completes off campus assessments for prospective residents.
  • For any licensed activity being supervised, maintains awareness of and compliance with all regulatory and licensure requirements; maintains good working relationship with inspecting and regulating entities.
  • Maintains and updates administrative records, quality management activities, personnel records, policies and procedure manuals for department.
  • Serves as an internal resource and consultant to management, and staff about home services for Posada Life at Home program.

Skills and Abilities

  • Education: Bachelor, Diploma, or Associate Degree in Nursing from an accredited institution or is an advanced degree nurse practitioner.
  • Experience: Minimum of 5 years of clinical experience, with 2 years of administrative, supervisory, or charge nurse experience including 2 years of home health or assisted living or nursing home experience. Must be familiar with Private Duty guidelines for Home Health.
  • Certifications and Licenses: Registered Nurse with a current license in Arizona. Current CPR and First Aid cards, Fingerprint clearance card, current TB test.
  • Computer Skills: Competent in the use of email and electronic medical records.
