Under the immediate supervision of the Director of Care Management Home Care, performs a wide range of home health services.
Essential Functions Statement(s)
Monitors resident condition by observing physical and mental condition, intake and output, and exercise.
Supports residents by providing housekeeping and laundry services; shopping for food and other household requirements; preparing and serving meals and snacks; running errands.
Assists residents by providing personal services, such as bathing, dressing, and grooming.
Helps residents care for themselves by explaining use of cane or walker, special utensils to eat, special techniques and equipment for personal hygiene.
Helps family members care for the resident by explaining appropriate ways to lift, turn, and re-position the resident; advising on nutrition, cleanliness, and housekeeping.
Records resident information by making entries in the resident journal; notifying nursing supervisor of changing or unusual conditions.
Maintains a safe, secure, and healthy patient environment by following asepses standards and procedures; maintaining security precautions; following prescribed dietary requirements and nutrition standards.
Updates job knowledge by participating in educational opportunities.
Protects La Posada by adhering to professional standards, home care policies and procedures, federal, state, and local requirements.
Prepares and serves simple modified diets according to instruction and assists with feeding as necessary.
Assists with toileting, including use of bedpan, commode or toilet.
Assists with transfers and ambulation including use of cane, walker, and wheelchair.
Assists with medication reminders as specified on plan of care.
Provides routine skin care.
Measures and records vital signs
Collects routine specimens.
Obtains residents weight
Assist residents in performing maintenance exercise programs.
Cares for male external catheter. Assists with the emptying of indwelling catheter care bag.
Assists the self-directing resident with the changing of a urinary diversion appliance or dressing when the ostomy is mature and stable.
Completes other duties as assigned within the scope of practice.
SKILLS & ABILITIES
Education: High School Graduate or General Education Degree (GED): Preferred
Experience: One-year recent experience as a CNA in a supervised setting or HHA
