We are currently seeking a weekend  RN Care Manager. This position is ideal for someone who is compassionate, dependable, working independently detail-oriented, prioritizes well, enjoys mentoring/educating others, values teamwork, and LOVES seniors. Experience in continuing care retirement communities, assisted living, memory care, or long-term care is a plus but we are willing to train the right applicant.

Key Responsibilities:

  • Reports problems and issues related to care, well-being, and health management of campus residents to the Director of Assisted Living and Care Management and At-Home Program and other RN Care Managers as appropriate.
  • Works collaboratively with team to develop departmental goals that promote La Posada’s mission, ends, and strategic goals, focusing on service excellence and exemplifying the values established by La Posada.
  • Collaborates with on campus and community staff involved in resident care to ensure the best care and services are provided to residents. Represents La Posada to the community in a professional and positive manner.

Requirements are:

  • Must hold a current Arizona RN license.
  • Minimum of five years of clinical experience.
  • Basic computer literacy.
